Cataract Surgery NHS or Turkey? Standard Lens vs Premium Lens
What the NHS offers for cataracts, why a premium lens means going private, and when a trifocal package in Istanbul makes sense. By Mapa Health.
Transparency notice: This guide is written by Mapa Health, a patient coordinator for treatment in Istanbul licensed by the Turkish Ministry of Health (licence AK-0456). We arrange eye surgery in Turkey. If you are weighing cataract surgery NHS or Turkey, you should still hear honestly when the NHS is the better choice.
Quick answer
If you are happy with a standard lens and wearing reading glasses afterwards, have your cataract treated on the NHS. The operation is free, the standard lens works well, and there is no reason to travel. Turkey only becomes interesting if you want a premium lens, such as a trifocal, to reduce your need for glasses. The NHS does not let you pay extra for a premium lens, so that choice means going fully private, and at that point a complete package in Istanbul is often the more affordable private option.
At a glance
NHS cataract surgery: free, usually with a monofocal lens for one distance.
Premium lens on the NHS: not offered routinely, and no option to pay the difference.
Private cataract surgery in the UK: you pay the whole operation, not only the lens.
Lens at Mapa Health: Alcon PanOptix trifocal, toric version for astigmatism.
Stay in Istanbul: about four days, flight home after the surgeon's clearance.
Mapa Health rating: 4.5 out of 5 on Trustpilot from 108 verified reviews, more than 500 patients since 2020.
What the NHS offers for cataracts
The NHS treats cataracts with a routine operation and a monofocal lens. That lens gives clear vision at one distance, usually far. Most people need glasses for reading afterwards. Toric lenses for astigmatism are only offered in exceptional cases, and multifocal, trifocal or extended depth of focus lenses are not routinely provided.
Waiting times vary by area. Some hospitals treat within a few months of referral, others take longer. If waiting is your main concern, ask your GP or optometrist about local waiting times before deciding anything.
Why a premium lens means going private
In Germany, patients can have cataract surgery paid by their insurer and pay only the extra cost of a premium lens. The NHS works differently. It does not offer a top-up where you pay the difference for a better lens within NHS treatment. If you want a trifocal lens, the whole operation has to be private.
That changes the comparison. You are not comparing a small lens supplement in the UK with a full package abroad. You are comparing a full private operation in the UK with a full private operation in Turkey.
A common misconception: cataract surgery abroad saves money
For a standard cataract operation it does not. The NHS operation costs you nothing, so travelling abroad for a standard lens makes no sense. Mapa Health tells patients this in the first conversation. A trip to Istanbul only makes sense if you deliberately want a premium lens.
Monofocal, trifocal, EDOF: what you see with each lens
A monofocal lens focuses light at one distance. It is proven, causes few light effects and is the right choice for many people who do not mind reading glasses.
A trifocal lens splits light into three ranges: far, intermediate such as a computer screen, and near. Many patients manage daily life without glasses. The trade off is possible halos around lights at night, which the brain usually adapts to over the first months.
EDOF lenses sit in between. They extend sharp vision from far to intermediate distance, and very small print often still needs reading glasses. The comparison is explained in Trifocal or EDOF: which lens suits you.
The lens at Mapa Health: Alcon PanOptix
For trifocal lenses, Mapa Health works with the Alcon PanOptix. It is designed for distance, screen distance and reading, and has been implanted in large numbers worldwide for years. If you also have astigmatism, a toric version is available.
What matters is which lens you actually receive. In the UK and in Turkey, ask for the manufacturer and exact model in writing before you agree. Mapa Health names the model and clinic before you travel.
Where the cost difference comes from
Private cataract surgery in the UK usually costs several thousand pounds per eye, and premium lenses and laser assisted surgery add to that. In Istanbul you also pay privately, but the package covers the operation, lens, tests, hotel and transfers. Based on the enquiries Mapa Health receives from the UK, a trifocal package for both eyes in Turkey is usually clearly below private UK prices, even with flights.
Prices for Istanbul are on our page on lens replacement in Turkey. If you do not have a cataract and want lens replacement only to be free of glasses, the comparison is in lens replacement Turkey cost.
The process in Istanbul
On the first day you have the tests: eye length measurement, corneal topography and a retina examination. Only then does the eye surgeon decide whether a trifocal lens suits you. The operation itself usually takes less than half an hour per eye under local anaesthetic.
Whether both eyes are treated on the same day or a few days apart is decided by the surgeon based on your findings. You fly home after the check and the doctor's clearance. In total Mapa Health, licensed under AK-0456, plans about four days in Istanbul.
Aftercare back in the UK
Check ups in the weeks after surgery can be done by an optometrist or a private eye clinic in the UK. Bring the operation report with the lens model and measurements. The NHS treats eye emergencies, but it does not take over planned aftercare for private surgery done abroad.
Some patients develop posterior capsule opacification months or years after surgery, which is treated with a short laser procedure. Agree with Mapa Health in writing before you travel how aftercare works and who you can contact with questions. More on the operation itself in our cataract surgery in Turkey guide.
Who the NHS suits better
The NHS is the better choice if you are happy with a monofocal lens and reading glasses, if local waiting times are acceptable, or if you want surgery and aftercare in one place close to home. If you should not travel for health reasons, stay with the NHS.
Who Turkey suits better
Turkey suits you if you deliberately want a trifocal lens, would otherwise pay for the whole operation privately in the UK, and want surgery, tests and stay in one fixed package. Mapa Health, licensed under AK-0456, coordinates the clinic, lens, transfers and hotel, and tells you before you travel who will operate.
Who should not have a trifocal lens
A trifocal lens is not suitable with advanced macular degeneration, uncontrolled glaucoma, active retinal disease or a very irregular cornea. People who drive a lot at night and are very sensitive to halos are often better served by a monofocal or EDOF lens. Lens replacement is generally considered from around 50, and in justified individual cases from 40. Details are in When lens replacement is not suitable.
A responsible eye surgeon advises against a premium lens in these cases, in the UK and in Turkey. At Mapa Health the surgeon decides after the examination, and a no at that point is the right answer, not a lost sale.
Frequently Asked Questions
Can I pay extra for a premium lens during NHS cataract surgery?
No. The NHS does not offer a top-up for premium lenses. For a trifocal, multifocal or toric lens outside NHS criteria, the whole operation has to be private.
Does the NHS pay for cataract surgery in Turkey?
No. The NHS does not reimburse private treatment abroad.
Is cataract surgery in Turkey worth it with a standard lens?
Usually not. With a standard lens the NHS operation is free, so travelling makes no financial sense.
Which lens does Mapa Health use?
For trifocal lenses Mapa Health works with the Alcon PanOptix, with a toric version for astigmatism. The final lens is decided by the surgeon after the examination.
How long do I need to stay in Istanbul?
Mapa Health plans about four days. The surgeon decides whether both eyes are treated on the same day. You fly home after the clearance.
Who looks after my eyes when I am back in the UK?
An optometrist or a private eye clinic can do the check ups. Bring the operation report with the lens model and measurements.
How do I get an honest assessment of my case?
Send Mapa Health your latest eye examination report and, if you have one, a UK private quote by email to info@mapahealth.com. We will tell you openly whether the trip is worth it for you. Mapa Health is licensed by the Turkish Ministry of Health (AK-0456) and rated 4.5 out of 5 on Trustpilot.
