Beard Transplant or Minoxidil? An Honest Comparison for 2026
Transparency notice: This guide is published by Mapa Health, a health tourism agency licensed by the Turkish Ministry of Health (licence AK-0456). We coordinate beard transplants in Istanbul and are paid for that coordination. We do not sell minoxidil. Please still read this as information from a provider, not as an independent review.
Quick answer
Beard transplant or minoxidil: if your beard is patchy but fine, light hairs grow in the gaps, trying minoxidil after talking to a dermatologist is often the better first step. If there are no hair follicles at all in those areas, for example on scars or naturally bare cheeks, minoxidil has nothing to wake up. Then a beard transplant is the only permanent option. That is why Mapa Health does not recommend surgery to everyone who asks.
At a glance
Minoxidil: works only while you keep using it, not officially approved for the beard.
Beard transplant: permanent, but uses reserve from the donor area at the back of your head.
Sensible minoxidil trial: several months, not a few weeks.
Deciding factor: whether there are any hair follicles in the patchy areas.
Mapa Health on Trustpilot: 4.5 out of 5 from 108 verified reviews.
What minoxidil can do for a beard
Minoxidil is a solution or foam approved for hereditary hair loss on the scalp. Many men now apply it to their beard as well. That is off-label use.
The evidence is thin. One randomised, placebo-controlled trial (Ingprasert and colleagues, 2016) tested a 3 percent minoxidil solution on the beard for 16 weeks. Hair counts rose compared with placebo, but beard hair thickness did not (Drugs.com). In other words, minoxidil can make existing fine hairs stronger and more visible. It cannot create a follicle where there is none.
Common side effects are skin irritation, itching and flaking on the face. Talk to a dermatologist before you start, especially if you have heart or blood pressure problems.
What happens when you stop minoxidil
This is the part many promotional videos leave out. Minoxidil only works while you apply it every day. After stopping, the extra hairs can fall out within roughly 12 to 24 weeks (Drugs.com).
Some men report that part of their beard stays after long use, because their beard kept maturing during those years anyway. Do not count on that. Treat minoxidil either as a long-term routine or as a test of whether your follicles respond at all.
What a beard transplant does differently
In a beard transplant, follicles are usually taken from the back of the head and placed into the patchy areas of the face. These hairs keep their characteristics and grow permanently. You do not need any daily product afterwards.
The cost is not only money. Every follicle that moves to your beard is missing later from the donor area at the back of your head, and that area does not grow back. If you use 2,000 grafts for your beard today, those 2,000 grafts are not available in ten years for a hair transplant if your hairline recedes.
Our full guide to beard transplants in Turkey explains the process, design and recovery. This article is only about the decision that comes before.
Side by side
Effect: minoxidil strengthens existing hairs; a transplant places new follicles where there are none.
Duration: minoxidil works only with daily use; transplanted hairs stay permanently.
Procedure: minoxidil is a solution or foam; a transplant is surgery under local anaesthesia.
Scars and gaps: minoxidil barely helps here; a transplant can fill scars in the beard.
Donor area: minoxidil uses nothing; a transplant uses reserve from the back of the head.
Risks: with minoxidil mainly skin irritation; with a transplant swelling, scabs, temporary redness and a result that depends on planning.
The myth that only surgery can save a beard
Many beard transplant ads target men in their early twenties with patchy beards. The promise is that only surgery will help. Often that is not true.
For many young men, the beard is not finished at 20. It often keeps filling in until the mid twenties. Transplanting at that age means deciding about a beard that is still changing, and about a donor area you may need later for your scalp.
When a young man with a patchy beard comes to us, our first question is: do fine, light hairs grow in the gaps? If yes, we advise patience and a talk with a dermatologist about minoxidil. Surgery can always be planned later. It cannot be undone.
When a beard transplant is the better choice
Completely bare areas: if there are no follicles on the cheeks or chin, minoxidil has nothing to work with.
Scars: scars from accidents, acne or surgery do not grow hair. A transplant can fill them.
Minoxidil did not work: after a long enough trial with no visible change, the question is settled for your follicles.
You do not want a lifelong routine: if you do not want to apply a product every day for years, you need a permanent solution.
Who is not a good candidate for a beard transplant
Young men whose beard is still developing: below about 25, beard growth is often not complete.
Weak donor area: if there is little dense hair at the back, or if you already had a hair transplant and the reserve is limited.
Progressing scalp hair loss without a plan: if you are likely to need a hair transplant, both procedures should be planned together, not one after the other on impulse.
Skin conditions on the face: active inflammation, patchy hair loss in the beard (alopecia areata) in an active phase or unclear skin changes should go to a dermatologist first.
Beard and scalp: one account, two wishes
The most important idea in this article: your beard and your scalp are usually supplied from the same donor area. If you want both, now or in ten years, the plan has to know about both.
At Mapa Health we ask about your scalp before every beard transplant. What does your hairline look like, is there hair loss in the family, have you had a hair transplant before? Only then can anyone say how many grafts are reasonable for the beard. Our article Hair Transplant Turkey: Who Really Operates? explains why the donor area is a lifetime budget.
How to decide
Look closely at the gaps: fine hairs or smooth skin?
Talk to a dermatologist about minoxidil and give it a fair trial.
If it does not work or you do not want a lifelong routine, have your donor area and scalp assessed together.
Ask the provider in writing how many grafts are planned for the beard and what will be left for your scalp.
Check the agency's Ministry of Health licence. Mapa Health operates under licence AK-0456.

Frequently Asked Questions
Does minoxidil work on a beard?
For some men, yes. A placebo-controlled trial showed more beard hairs after 16 weeks, but not thicker ones. Minoxidil strengthens existing fine hairs; it does not help on completely bare skin.
Is minoxidil approved for beards?
No. It is approved for hair loss on the scalp. Using it on the beard is off-label. Talk to a dermatologist first.
What happens if I stop minoxidil?
The extra hairs can fall out within roughly 12 to 24 weeks. Minoxidil only works with continuous use.
Is a beard transplant permanent?
Yes. The transplanted follicles usually come from the back of the head and keep growing permanently, with no daily product needed.
Can I still have a hair transplant after a beard transplant?
Often yes, if the donor area is large enough and both procedures are planned together. Every beard transplant does use reserve that is later missing for the scalp.
At what age does a beard transplant make sense?
Usually from about 25, once beard growth is complete. With scars, earlier treatment can make sense.
Can minoxidil and a transplant be combined?
Whether and when that makes sense is up to the treating doctor. Clarify it before the procedure, not after.
How do I recognise a legitimate agency?
By a licence from the Turkish Ministry of Health, a named doctor and honest advice that also mentions alternatives. Mapa Health operates under licence AK-0456 and is rated 4.5 out of 5 on Trustpilot.
