Which material is right is decided not by a package but by the condition of each individual tooth.

What is an E.max veneer?

E.max is the brand name of a lithium disilicate glass ceramic made by Ivoclar. The material lets light through much like natural enamel. That is why it looks especially natural on the visible front teeth.

A veneer is a thin shell that covers only the front of the tooth. The rest of the tooth, the back and the sides, stays untouched. This is the main advantage: an E.max veneer changes the shape, colour and length of a tooth without sacrificing much healthy structure.

The precondition is a healthy, well preserved tooth. Veneers are bonded to enamel. If little enamel remains or the tooth is heavily filled, a veneer holds less reliably.

What is a zirconia crown?

Zirconia, or more precisely zirconium dioxide, is a very strong high-performance ceramic. A zirconia crown encloses the tooth completely, like a cap. It protects and stabilises teeth that are already weakened.

Modern monolithic zirconia looks much more natural than earlier generations. It remains slightly less translucent than E.max, however. With heavily discoloured teeth this is even an advantage, because zirconia masks dark discolouration more reliably.

For a crown, the tooth has to be reduced on all sides so that the crown fits and does not look bulky. That is why zirconia is the right material for teeth that need a restoration anyway, and not for healthy teeth that only need to look brighter.

How much is filed down?

This is the question patients ask most often, and the difference is clear.

With an E.max veneer, usually only a thin layer is removed from the front of the tooth, typically a few tenths of a millimetre. The tooth keeps its shape and is only prepared on the visible surface.

With a zirconia crown, the tooth is reduced on every side, often by a millimetre or more. What remains is a smaller tooth stump onto which the crown is placed.

Neither can be reversed. Enamel does not grow back. That is why every preparation should be preceded by a digital smile design and, where possible, a trial in the mouth with a temporary material. Only once you have seen and approved the shape, length and shade is the tooth prepared.

This is the core of what is often called Turkey teeth: in many of these cases healthy teeth were filed down for crowns although veneers would have been enough. The problem is not the country but the wrong choice of material.

Which tooth gets which material?

Durability, care and teeth grinding

With good care and regular check-ups, both materials often last ten years or longer. How long exactly depends less on the material than on three things: oral hygiene, healthy gums and your bite.

The biggest risk factor is grinding your teeth at night. It puts heavy strain on veneers and crowns and can cause chipping. Anyone who grinds should wear a night guard after treatment.

Care is the same as for natural teeth: brush twice a day, use floss or interdental brushes and have a professional clean twice a year, ideally with your dentist at home.

A written 5-year commitment for material defects

For veneers and zirconia restorations coordinated through Mapa Health, the treating partner clinic provides a written 5-year commitment covering material and workmanship defects. One to two years is usual in the industry.

Covered are, for example, a fracture or debonding caused by a material or workmanship defect. Who should not have veneers or crowns

An honest coordinator also says when neither veneers nor crowns are the right answer.

With untreated decay or inflamed gums, the underlying condition is treated first.

If you only want whiter teeth, professional whitening is often the better choice.

With crooked or crowded teeth, treatment with clear aligners often gives a better result without removing tooth structure.
